Discharge Planning

Discharge Planning

Every hospital or chronic care facility has a staff charged with the responsibility of arranging for a safe discharge once a patient no longer requires the level of care provided by a hospital or acute care facility. There are circumstances which may call for the appointment of a guardian, or the patient's family may be unwilling or unable to provide an acceptable level of care. At these times our attorneys are needed to affect a safe and timely discharge.
